MTLPS Intent to Return ~ 2018-2019 Preschool Year

This form is for currently-enrolled families. Please fill out the form below with your intentions for next school year. If you are enrolling siblings, submit separate forms for each child.

Classes and schedules will be assigned in May, but are not guaranteed until student 2018-19 Activity Fee deposit $110 is received no later than June 10, 2018.
Please refer to tuition agreement regarding vacation and travel.

Please be reminded this form is used for our planning, staffing, and hiring. If a family chooses to add a day we will work hard to accommodate additional days if given enough notice. However, once processed, a family must get director approval to drop days.
